Barriers to Health Care For Teen Pregnancy

Although teen pregnancy rates have been declining in the past, it is still a trend that is disturbing. With some having sex as early as 11-12 years of age, the chances of a teen pregnancy are increased. With teen pregnancy comes the need for health care. Unfortunately, there are many barriers to health care for a teen pregnancy.

Education is a must

One of these barriers to health care for the pregnant teen is education. Schools do not educate students about their bodies and the way they work. Parents tend to be uncomfortable about discussing sex education matters with their teens, so any information the teen may get usually comes from their peers. This is not usually an effective means of gathering correct information.

Financial Barriers

Another barrier to health care for teen pregnancy is income, or the lack of it. Health care is prohibitively expensive for many adults, and even a teen with working parents may not have access to health insurance. When a parent has to decide between rent and food or health insurance, the medical needs are often overlooked. With access to education about pregnancy, and clinics available for teens with no health insurance, there might be a rise in teens seeking out medical care for themselves and their unborn child.

Along with inadequate health insurance and low income parents, teens may face lack of transportation to a health care facility. Many 2 income families have one car, and no support system to turn to in times of need. Often health care is not available within a reasonable distance, or there may be no gas for the second car. Even if there is transportation available, a parent may not be able to get time off from work to take the pregnant teen to the doctor, providing the parent is aware of the pregnancy in the first place.

Shame and Fear

This brings us to another barrier to health care for teen pregnancy. Shame and fear in a teenager can be a powerful motivator for many actions they take. Shame for being pregnant, and fearful of the consequences they may face from a parent can influence a teenager in many ways. One coping skill that stems from these feelings is not admitting to the pregnancy, even to themselves. This, of course, can lead to the teen not receiving any health care until she has progressed far into her pregnancy. This only sets the teen up for possible complications down the road for her and her baby.

With education of parents and teens together, perhaps we can overcome these barriers to health care for our pregnant teens. Communication for all can be a good first step in providing access to health care for both the mother and her unborn child.

5 Components of Physical Fitness

While the definition of physical fitness can be a little complex or unclear and the definition of physical fitness can vary, most government health agencies and exercise scientists agree that there are 5 components of physical fitness related to health. These components provide a fairly accurate representation of how fit and healthy the body is as a whole (total or overall fitness). The 5 components are cardiovascular fitness (also referred to as cardio-respiratory endurance or cardiovascular endurance), muscular strength, muscular endurance, flexibility, and body composition.

Let’s take a closer look at these components individually.

1.) Cardiovascular fitness (or cardio-respiratory endurance or cardiovascular endurance)

Of the 5 components, cardiovascular fitness is the cornerstone that creates the pathway to improving your other fitness levels.

Cardiovascular fitness is the efficiency with which the body (the heart and lungs) delivers oxygen and nutrients to the required working muscles and transports waste products from the cells over a sustained period of time. Or to put it another way, it’s the ability of your heart and lungs to work together to provide the necessary oxygen and fuel to your body without quickly reaching a high level of fatigue and tiredness.

In our daily lives, we need cardiovascular fitness to handle the physical tasks and all of the “running around” we do.

A common test of cardiovascular fitness usually involves some type of sustained running. But typical examples of physical activities that relate to cardiovascular fitness are jogging, swimming, cycling, brisk or speed walking and any type of aerobic exercises. Aerobic exercise is the best way to improve cardiovascular fitness.

2.) Muscular strength

Muscular strength is the maximum amount of force (weight or heavy resistance) a muscle or muscle group can generate in a single effort to the point that no more repetitions can be done without rest. Muscular strength is quite the opposite of cardiovascular fitness in regards to the fact that cardiovascular fitness is measured over a certain period of time. While on the other hand, muscular strength is measured in one repetition.

In our daily lives, we need modest levels of strength to be able to perform everyday physical tasks like lifting, moving, carrying, etc.

A common test to measure upper body strength is some type of weightlifting exercise, such as the bench press. Anaerobic weightlifting exercises like the bench press, leg press, shoulder press, or bicep curls are examples of the best ways to improve muscular strength.

3.) Muscular endurance

Muscular endurance is the ability of a muscle or group of muscles to perform repeated movements (or to hold a particular position) with less than maximum force for an extended period of time or until muscular fatigue. Or, to put it simplistically, it’s how long your muscles can do something before getting too exhausted to finish.

Be careful not to confuse muscular endurance with muscular strength. While they can work together, they are definitely not the same. For many athletes, there may be a need to distinguish between muscular strength and muscular endurance. But for everyday people who want to easily perform their daily routines, are trying to stay healthy and fit, and just want to enjoy physical activities like hiking, biking, or just playing in the park with their children, muscular endurance plays a major role in fitness.

Common testing for muscular endurance can be dynamic (the ability to repeat contractions) or static (the ability to sustain a contraction). Dynamic tests would be to see how many push-ups or sit-ups, for example, a person can complete in a designated amount of time (e.g. 30 seconds, a minute, or maybe longer). Or, without being timed, the person could do as many repetitions of the exercise as they could until they couldn’t do anymore. An example of a static test would be the flexed-arm hang whereby the performer hangs on a bar until the designated stopping time or until they become too weak to continue hanging.

Muscular endurance can be improved by both aerobic and anaerobic exercises. Some examples would be biking, step machines and elliptical machines.

4.) Flexibility

Flexibility is the ability to move the joints or any group of joints, muscles, ligaments, and tendons through their full, normal range of motion without hindrance, discomfort, or pain.

Flexibility is actually more important to physical fitness than people realize. Not only does flexibility play a big role in performing many daily tasks, but maintaining or even increasing your flexibility is critical to protecting your joints and keeping them healthy. In addition, being flexible contributes to improving your lower back health, reducing the appearance and effects of arthritis, and reducing muscle-tendon injuries.

I start my day by hitting the gym for an intense workout session; I do strict diets hoping to shed some weight. I swear I do not do this because my body doesn’t impress me, but because it doesn’t impress the society. Every day is a struggle – a neighbor, a relative and sometimes even a friend reminds me that I’ve gained a lot of weight, forcing me to fight with my body.

Do I enjoy it? Of course not and I’m sure anybody who is exercising or dieting under pressure isn’t enjoying any bit of it.

I’ve been through a lot, I’ve heard people say mean stuff about me and stare at me, but nothing bothers me now! I’m happy to be the ‘fat woman’ that I am, and if you’re just as fat like me, you should be happy too. Stop blaming others for how you feel. Remember what Eleanor Roosevelt said? She said, “No one can make you feel inferior without your consent.” So sorry to burst the bubble girls but it’s not the others who mocked you, it was you who degraded yourself and gave them the liberty shame you.

I understand and agree that a ‘thin and lean body’ with the ‘perfect abs’ has become the new ‘vogue’, but this does not mean that you have to fit into the norms set by the society. Let people call you fat, curvy, fuller, plus size, obese, chubby or with any other similar word that suits their taste. It is okay to exercise if you want to but don’t starve yourself to death because someone else told you. Give yourself treats that you love; it’s okay to munch a cheese burst pizza along with some fries, and chocolate if you want, or you can just enjoy some juice if that pleases you. Remember the life that you’re leading is your own and no one has the right to judge you on the basis of your size, the clothes that you wear or the food that you eat. Relax!

As for me, I learned things the long way – it took me several years to accept my ‘fat body’ and to understand that there is no definition of a perfect body. Whether I’m fat or lean people will always find their reasons to call me names.

The kind of negativity revolving around plus size doesn’t irritate me. I’m a believer of self-love, my size doesn’t bother me any longer. To the people who’re over sympathetic for me, I’d like to say, “I don’t care. Stop wasting your time here!”

Believe me when I say you’re going to feel extremely confident and comfortable if you learn to wear your curves proudly. Remember you’re a healthy beautiful woman and nobody can take that away from you.

What this means is that the health coach can identify with the feelings, thoughts, or attitudes of his client. Using empathy may lead to an improved relationship between the health coach and client.

Another quality that is related to empathetic communication is to really listen, actually to listen effectively. A coach needs to be trained to listen effectively since people vary in their communication skills. Some clients may express themselves clearly while others may not. A coach needs to be aware of the different needs of his/her client base and how the coach/client communication and listening skills affect the interaction.

The wellness coach has to leave his personal opinions out of the coaching session. The coach needs to be completely non-judgmental. The wellness coach absolutely should not judge his/her client based on his personal standards or opinions. Much of a coach’s training will be on how to be aware of this and to have the self control to NOT allow his personal opinions to be communicated.

Another fundamental quality of a health coach is the integrity to keep all information about the client confidential. The coach/client relationship is based on trust and the client must be sure that the coach can be trusted with private matters.

In addition, the coach still must be somewhat of a mentor, guidance counselor and cheerleader all wrapped up in one! The health coach should allow the client some flexibility in the agenda of each session, but the coach should also guide the session conversation so that it is an effective one. The client sets his/her own goals, again with the guidance from the coach. The coach may challenge the client a bit if the goals are too vague or too big. In the latter case, the coach may suggest smaller “bite size” goals so that the client has a better chance of reaching those goals. Of course, all successes are celebrated.

In summary, there are many qualities that a health coach should have and there are also many roles that he/she has. Each coaching relationship will be different and the wellness coach will need to be a bit flexible. A coaching relationship can in some cases grow into a life-long friendship as well.

New York Passed and Then Repealed Medicaid Expanded Estate Recovery: A Recap

What is “estate recovery”?

When a Medicaid-recipient dies, Medicaid may seek reimbursement from his or her estate for the benefits paid on his or her behalf. This is known as “estate recovery”. Each state has rules detailing how, and to what extent, such recovery is possible.

Historically, NY limited estate recovery to probate assets

Until 2011, New York limited such recovery to the Medicaid-recipient’s probate assets — i.e., only those assets titled solely in the Medicaid-recipient’s own name. Other assets — including assets held jointly with another person, a “life estate” reserved in a deed, and assets held in revocable and irrevocable trusts — were excluded from estate recovery.

New rules and regulations expanded estate recovery

On April 1, 2011, New York enacted “expanded” estate recovery rules[1]. These new rules were made subject to regulations that were to be enacted by the New York State Department of Health, and on September 8, 2011 the Department of Health finally enacted these much-anticipated regulations[2]. The regulations were actually enacted as “emergency regulations”on an expedited basis, with a built-in expiration date if not extended or otherwise converted into permanent regulations. then, on September 26, 2011, an Administrative Directive was issued by the New York State Department of Health to clarify the scope of the regulations[3].

These new rules and regulations expanded the definition of “estate” to specifically include as assets subject to estate recovery those owned by the decedent “through joint tenancy, tenancy in common, survivorship, life estate, living trust or other arrangement, to the extent of the decedent’s interest in the property immediately prior to death” (emphasis added). Both the expansion into these categories of assets, and the express wording “immediately prior to death”, were quite significant. As explained below, they were also very troubling.

For example, a very popular planning tool is the use of a life estate in a deed for the primary residence. It is a powerful tool because it accomplishes a number of things, notably: (1) it removes the property from the Medicaid-applicant’s name; and (2) the life estate causes the tax basis in the real estate to be “stepped up” to the Fair Market Value upon the Medicaid-recipient’s death, thereby saving tens of thousands — even hundreds of thousands — of dollars in capital gains tax once the Medicaid-recipient dies. As you might imagine, the life estate deed — which is relatively easy, quick, and inexpensive to implement — has been very popular and widespread as a planning tool.

In New York, the theory was that a life estate extinguishes at the moment of death, so that there is nothing for Medicaid to put a lien on or pursue after the Medicaid-recipient’s death. But once the definition was expanded to include life estates and the value of the life estate “immediately prior to death”, Medicaid intended to pursue recovery against the life estate. Consequently, whereas before the critical calculation was the value of the life estate based on the life expectancy of the recipient at the time of the deed transfer, now the key concern became the amount recoverable, which is the value based on the Medicaid-recipient’s age just prior to the moment of death.

The problems with the new rules and their enforcement

Right from the start, the new rules were opposed based on constitutional and other legal grounds — in fact, the New York State Bar Association filed a lawsuit challenging them. Among the problems with the new rules:

1. There was no clear effective date and no grandfathering. Children who had real estate transferred to them by their parents years ago could have faced the dilemma of reimbursing the State for care provided to their parents decades after the transfer was done, even if the property was no longer owned by the family and even if the proceeds (if any) were long gone. Similarly, the parents and others who did their planning under the laws that were in effect at the time of the transfers, could have had all their planning — even if done decades ago — unraveled by the new law;

2. The duration of the Medicaid Lien was not stated;

3. Title companies could have had many problems and possible financial exposure stemming from the ensuing title issues.

Due to the pressure from the Bar Association, and the problems and inconsistencies pointed out, the regulations were not extended, were not made permanent, and instead were allowed to expire after December 6, 2011[4].

Expanded estate recovery is repealed

Finally, on March 27, 2012, New York repealed[5] the regulations that had expanded the definition of “Estate” for Medicaid recovery purposes; therefore, the old rules governing estate recovery now remain in effect and life estates are no longer vulnerable to recovery.

Create a vision board by taping these pictures to a piece of paper and putting them above your desk or by your bed, or anywhere where you can gaze at them often. Each day, take some time to look at your vision.

Step Three: Send out a Request to the Universe

This is about connecting with a force greater than yourself to ask for what it is that you want. It might be difficult to ask for what you want, some might not feel like they deserve it, but the universe, will always support your requests that are for the greater good. Requests about being slimmer, healthier and happier will help each individual to feel better about themselves and to feel happier. This in turn creates more happiness around that person thus creating a more positive world. It’s the small things that build up.

Step Four: Write it Down

Writing is a very powerful and immediate way to bring about change. The following will help you to stay on track with your weight loss. First, buy a law of attraction weight loss journal. Chronic and Acute Inflammation – Prevention and Treatment

Some of my patients have been asking me recently, about a new medical condition, inflammation, that’s being investigated as the underlying cause of fatigue and most diseases including, cancer and heart disease. As I tell my patients, the truth is, inflammation is not something new, though our unhealthy diets and lifestyles may be adding to the rise in its occurrence. Inflammation, and its ill effect on health, have been around for centuries, and have been well studied and treated by alternative, non-Western medicine, for just as long. Seems Western medicine is just starting to catch on to damaging, chronic inflammation.

Inflammation, or yang, as it’s called in Asian medicine, is one of the pernicious influences in human health. Together with it’s opposite; the dampness/coolness of yin, Asian medicine believes that these two forces form the root of all diseases. However, you don’t have to go to the Far East to get rid of inflammation as the treatment is the same in Western medicine, i.e., re-balance the blood and tissues to cool the inflammation. Allow me to tell you a little about this condition and share with you some simple things you can do to rebalance your yang force and obtain better health.

Inflammation, Friend and Foe

Inflammation can help us and hurt us. Acute inflammation is the beneficial side of the condition that occurs when we injure ourselves through a cut, bruise, sprain or fracture. The surrounding area turns red and may become hot to touch. This is our immune system’s first defense attempt at healing the damaged part. It sends white blood cells to the area which causes the tissues to heat up in order to kill an infection that may develop from the injury.

On the flip side, chronic, low-grade, ongoing inflammation, can be damaging to our overall health. Somehow, the body’s immune system just keeps pumping out inflammatory responses and instead of healing the body it starts to make it ill. But how does this happen? Well, it can come about from a variety of factors, the following being the most common:

  • Smoking
  • Too much alcohol
  • Too much refined sugar, high glycemic carbohydrates
  • Insufficient good fatty acids (Omega 3’s) in diet
  • Insufficient antioxidants
  • Too little exercise, overweight
  • Diabetes

In short, the immune system starts to think of itself as under constant attack from poor diet and lifestyle habits and works overtime. In fact, many researchers now believe that inflammation may be the key to understanding how all diseases start, from Alzheimer’s, asthma, arthritis, GERD and ulcers, to heart disease and even cancer. Interestingly, treating these diseases successfully may also lie in treating the inflammation that’s fueling them.

How Can You Get Rid of Inflammation?

In order to work on reducing inflammation, you first have to know if you have elevated levels of certain tests that are excellent markers for inflammation. These are:

  • CRP – short for C-reactive protein, elevated in chronic inflammation.
  • Homocysteine – elevated, usually from a lack of folic acid and/or too high iron in diet.
  • MPO – short for myeloperoxidase. A relatively new test for inflammation can help determine potential cardiac risk factors such as atherosclerosis.

Next, do everything possible to stop any inflammation-producing activities. For example, quit smoking, cut down on alcohol or limit it to special occasions. Get some exercise. People who exercise frequently tend to weigh less and have lower CRP, homocysteine and MPO levels. Reduce your weight as fat cells are filled with cytokines which boost inflammation. Get tested for diabetes. Untreated, uncontrolled diabetes can fuel inflammation, damage blood vessels, and put you at increased risk for heart disease.

Follow an inflammation-fighting diet such as the following:

  • Limit sugar/refined carbohydrates. Cakes, pies, candy, some fruits, and high glycemic carbohydrates (bread, pasta), all create a high acid condition which fuels inflammation.
  • Eat more good fats. Add high monounsaturated fats like mixed nuts, avocado, olive oil, as well as fatty fish like salmon, tuna, mackerel, sardines a few times a week. If you eat canned tuna, choose water packed as the Omega-3’s leach into the oil packed varieties that will get drained off before eating. Or, take two 1,000 mg fish oil capsules a day.
  • Limit acid foods. High acid foods like animal meats, eggs, lemon, grapefruit, and tomato.
  • Eat more alkali foods. Yellow, dark green, vegetables like yams, kale, cucumbers fight inflammation by balancing acid with alkali.
  • Add antioxidants. Our modern diet can’t possibly contain enough antioxidants to do us much good fighting inflammation. Add a good supplement that contains Vitamin C, resveratrol, E, selenium, Vitamin D3, bilberry, blueberry. Spices like turmeric (curcumin), ginger, cayenne, parsley.
  • Increase fiber. Fiber helps sweep inflammatory toxins out of your system.
