Concierge Medicine – Is it For You?

The last time you called your doctor’s office, how long did you have to wait for a reply? How long do you usually have to wait to get an appointment or wait for a prescription to be called in? When you get to the office, how long do you spend in the waiting room and how much time does the doctor spend with you?

Did you get all your questions answered? How many seconds do you have in the beginning of your appointment to explain your symptoms before you are interrupted? Do the nurses and doctors seem to be more interested in their computers or their charts than they are to you?

If your experiences are like most people, your answers to these questions are not very flattering to the medical profession and to the health care system in general. Most doctors don’t really want their practices to be like this, but they don’t have much choice. They have to have a high volume of patients in order to make ends meet financially. The high volume makes the clinic a very busy place and most patients don’t feel like they get much attention.

In 1996 in Seattle, a doctor named Howard Moran thought there should be a better way to do this. He pioneered the concept of having a lower volume practice with highly attentive medical care provided as a service for patients in return for a retainer fee, much like many attorneys or accountants use. This fee may be in addition to, or in lieu of, the regular office fees that are billed to insurance companies. This concept allows the practice to remain financially solvent while providing better, more attentive medical service to its low volume of patients (usually keeping the patient count down to about one tenth of the number in a typical traditional primary care practice).

Unfortunately, health insurance companies currently don’t pay for this type of service, so that means the patients have to pay this out of pocket, but if the service is good, it may be worth it. Patients who join these practices are encouraged to keep their usual insurance which they will need for visits to other specialists, laboratory testing, radiological testing and/or hospital services if needed.

Many concierge practices offer same or next day appointments, no long waits for appointments or prescription refills, direct access to your personal physician day or night, house calls if necessary, continuing care if hospitalized, complete physical examinations, audiometry screening, cardiovascular and cancer risk screening, more attention to preventive care, unrushed appointments, all questions answered, family meetings if needed, coordination of care with specialists, provision of personal health records on CDs or flash drives, etc., etc.

Depending on the type and number of services that are provided, the flat retainer fee can vary widely from one area to the next ranging from $100 to $20,000/ year, most probably averaging around $1500-3000/year. There were only a few hundred of these physicians a few years ago, but there are over 5000 of them now all over the country. Many of them, but not all, are listed at the main website for the American Academy of Private Physicians – http://www.aapp.org/. Patients may be able to find a concierge physician in their area on this site.

There has been some controversy surrounding some of these practices because some authorities feel the growth of concierge medicine will lead to a 2-tiered medical system in this country – one for the wealthy and one for the not-so-wealthy. Also, with the current shortage of primary care physicians (PCPs), with increasing numbers of concierge practices, the number of PCPs available for traditional offices will be even smaller, and access to care for patients served by those practices will suffer. On the other hand, more medical students may become interested in primary care if opportunities in concierge medicine are available to them when they finish their residencies.

Some of these practices have come under fire from insurance companies who say that concierge physicians are basically providing an insurance plan for their patients without having an insurance license to do so. The practices must therefore be legally and financially structured in such a way that avoids this criticism.

Overall, the concept of concierge medicine provides a good alternative to the currently unsatisfying traditional model of medical practice. The fee may seem high to some, but in most instances, it probably isn’t much higher than the cost of the local cable or telephone bill, or about the same as what a family would spend eating out at a restaurant once a week.

Jock Itch or Herpes? Here’s How to Tell the Difference

A persistent groin itch is enough to make a sane man crazy, as part of the brain is always resisting the urge to scratch. Men with itchy, sore genitals might also be frightened of the cause of the itch, worried that the symptoms they’re experiencing might be due to something chronic, like herpes. While herpes is common, some cases of crotch itching are caused by more benign conditions, including jock itch. Jock Itch: Causes and Symptoms

Jock itch can spread to the penis, but that’s not common. Instead, this rash tends to focus on the legs and the dark crevices of the body. Herpes, on the other hand, can infect the penis itself, and sores on the head of the penis are quite common. The itching symptoms caused by the two conditions can be quite similar, but the areas in which the itching takes place tend to be quite different.

The appearance of the two conditions is also strikingly different. Where jock itch tends to look a lot like a red rash, with a few tiny blisters on the edge, herpes infections look like heat blisters that are filled with fluid. When these blisters pop, they form painful, crusty sores. Jock itch doesn’t behave in this way.

Getting Better

Rashes and sores of any kind should be brought to the attention of a doctor. Medical professionals can use laboratory tests to determine exactly what is causing the itch, and they can prescribe appropriate treatments to keep that problem from spreading. Any man with an itch should put a call to the doctor at the top of his to-do list. With proper diagnosis, these self-care tips can help to speed healing.

1. Gabriel means: “God is my strength” or “God is mighty.” Gabriel is one of only two angels mentioned by name in the Bible. Michael is the other. Raphael is mentioned in the Apocrypha, which is included in the Catholic Bible.

2. Gabriel appears in the Bible as a messenger on important occasions. The most famous example of this is the Annunciation when he visited the Virgin Mary and told her to prepare for the birth of Jesus Christ (Luke 1:26-38).

3. Gabriel has always had a close association with maternity. An ancient legend says that she announced the birth of Samson. She appeared to Zacharias to tell him that he and his wife Elizabeth should prepare for the birth of a son, John the Baptist (Luke 1:5-25). In the Jewish tradition it is believed that Gabriel instructs the baby while it is in the womb.

4. Although not identified by name, many Christians believe that Gabriel announced Christ’s birth to the shepherds, warned Mary and Joseph that Herod’s soldiers were searching for Jesus, and rolled away the stone that sealed the tomb of Jesus.

Healthy Spring Water And The Value Of Proper pH Balance

What are our basic needs?

Oxygen (air)

Food

Water

Constant body temperature (98.6 F.)

Food is a need that is bought with money or time, so we can realize immediate value in it.
Body temperature has to be maintained by mechanical means, ie heat in winter, air conditioning in summer. These mechanical means cost initially on the front end and cost to maintain them, as we are aware of when we receive our utility bills.

Lets focus on the two basic needs that are often taken for granted, maybe because we do not immediately see value in them.

Air is, well, (for the time being, and the most part) still free. I suppose if we have to air up our auto tires, while on the road, we see immediate value in air. Then some Ecology students or Al Gore would discuss with us how much value we derive from the air we now breathe freely, and for how long we will be able to breathe this reliably clean air. Then for those of us who live in major metropolitan areas, the issue of clean air and the value of it is very real.
Let's talk about Free Air later.

Instead let's focus on Free Water, and the value of it. Did my Grammar School Teacher coin the phrase, "You can lead a horse to water, but you can not make him drink"; Or was she borrowing a quote from a timeless philosopher? What ever the case may be, the saying still remains true today and is relevant to this essay. If I were to take you by the hand and show you a cool, pure spring of water, maybe a natural mountain spring, a healthy spring, would you drink from it? Some would probably want it tested (a very good point). For the most part, though, this healthy spring is free. We would really see value in it if it were tested. Tested for what?

Well, let's back up a little and see where I came up with our four basic needs, no it was not from my High School Biology teacher, although I'm sure they led me to that knowledge, I just did not drink.

Abraham Maslow (1908-1970): PhD. In Psychology who created the now famous, "Hierarchy of Needs", labeled our 4 basic needs as the "physiological needs" and and are the foundation of all our need. Yes there are more, but the basics must be met before we advance to the others. Dr. Maslow identified the physiological needs as, oxygen, water, protein, salt, sugar, calcium, and other minerals and vitamins. They also include the need to maintain a pH balance (getting too acidic or base will kill you) and temperature (98.6 or near to it). Also, there's the needs to be active, to rest, to sleep, to get rid of wastes (CO2, sweat, urine, and feces), to avoid pain, and to have sex. Quite a collection!

I said (well, I and Dr. Maslow) all that to say this. Remember, I'm backing up and asked the question, "tested for what." Well Doctor Maslow listed all kinds of ingredients, I assume for the food group of which included minerals and proper pH balance. So then if minerals and a proper pH balance is essential to our basic needs, we would want to test that water for the same. Then if I pulled out my handy swimming pool water test kit and the healthy spring water tested no chlorine, not acidic, perfect pH balance, and I found a good mix of trace minerals, could we see value in it? Forgive me if I answer, sure we would see value and that healthy spring water would be desirous to drink.

Let me conclude this theses on a couple of points; Dr. Maslow died a reliably young man, perhaps he lost focus on his theorizing. One article stated he embarked on a crusade into "Humanistic Psychology". For whatever reason, he died at age 62, he is not a very good reference for my next point.

The Bible states in Gen. 6: 3 mans' days shall be one hundred and twenty years. While we seem to be doing better, the life expectancy in the late 1800's was like 45 yrs., And is now around 75 yrs., Very few Americans live to see 120, and if some reach 100, their quality of life is badly degraded . This is unlike most Japanese men, who live near the reefs of Okinawa.

The KWP2000 Protocol in Automotive Diagnostic Applications

The KWP2000 protocol has become a de facto standard in automotive diagnostic applications. It is standardized as ISO 14230-3. KWP2000 describes the implementation of various diagnostic services you can accethrough the protocol. You can run KWP2000 on several transport layers such as K-line (serial) or CAN.

Transport Protocol

As KWP2000 uses messages of variable byte lengths, a transport protocol is necessary on layers with only a well defined (short) message length, such as CAN. The transport protocol splits a long KWP2000 message into pieces that can be transferred over the network and reassembles those pieces to recover the original message.

KWP2000 runs on CAN on various transport protocols such as ISO TP (ISO 15765-2), TP 1.6, TP 2. 0 (Volkswagen), and SAE J1939-21. For KWP2000, the Automotive Diagnostic Command Set supports only the ISO TP (standardized in ISO 15765-2) and manufacturer-specific VW TP 2.0 transport protocols.

Diagnostic Services

The diagnostic services available in KWP2000 are grouped in functional units and identified by a one-byte code (ServiceId). The standard does not define all codes; for some codes, the standard refers to other SAE or ISO standards, and some are reserved for manufacturer-specific extensions. The Automotive Diagnostic Command Set supports the following services:

• Diagnostic Management

• Data Transmission

• Stored Data Transmission (Diagnostic Trouble Codes)

• Input/Output Control

• Remote Activation of Routine

Upload/Download and Extended services are not part of the Automotive Diagnostic Command Set.

Diagnostic Service Format

Diagnostic services have a common message format. Each service defines a Request Message, Positive Response Message, and Negative Response Message. The Request Message has the ServiceId as first byte, plus additional service-defined parameters. The Positive Response Message has an echo of the ServiceId with bit 6 set as first byte, plus the service-defined response parameters.

The Negative Response Message is usually a three-byte message: it has the Negative Response ServiceId as first byte, an echo of the original ServiceId as second byte, and a ResponseCode as third byte. The only exception to this format is the negative response to an EscapeCode service; here, the third byte is an echo of the user-defined service code, and the fourth byte is the ResponseCode. The KWP2000 standard partly defines the ResponseCodes, but there is room left for manufacturer-specific extensions. For some of the ResponseCodes, KWP2000 defines an error handling procedure. Because both positive and negative responses have an echo of the requested service, you can always assign the responses to their corresponding request.

Connect/Disconnect

KWP2000 expects a diagnostic session to be started with StartDiagnosticSession and terminated with StopDiagnosticSession. However, StartDiagnosticSession has a DiagnosticMode parameter that determines the diagnostic session type. Depending on this type, the ECU may or may not support other diagnostic services, or operate in a restricted mode where not all ECU functions are available. The DiagnosticMode parameter values are manufacturer specific and not defined in the standard. For a diagnostic session to remain active, it must execute the TesterPresent service periodically if no other service is executed. If the TesterPresent service is missing for a certain period of time, the diagnostic session is terminated, and the ECU returns to normal operation mode.

GetSeed/Unlock

A GetSeed/Unlock mechanism may protect some diagnostic services. However, the applicable services are left to the manufacturer and not defined by the standard.You can execute the GetSeed/Unlock mechanism through the SecurityAccess service. This defines several levels of security, but the manufacturer assigns these levels to certain services.

Read/Write Memory

Use the Read/WriteMemoryByAddress services to upload/download data to certain memory addresses on an ECU. The address is a three-byte quantity in KWP2000 and a five-byte quantity (four-byte address and one-byte extension) in the calibration protocols. The Upload/Download functional unit services are highly manufacturer specific and not well defined in the standard, so they are not a good way to provide a general upload/download mechanism.

Measurements

Use the ReadDataByLocal/CommonIdentifier services to access ECU data in a way similar to a DAQ list. A Local/CommonIdentifier describes a list of ECU quantities that are then transferred from the ECU to the tester. The transfer can be either single value or periodic, with a slow, medium, or fast transfer rate. The transfer rates are manufacturer specific; you can use the SetDataRates service to set them, but this setting is manufacturer specific. The Automotive Diagnostic Command Set supports single-point measurements.

Diagnostic Trouble Codes

A major diagnostic feature is the readout of Diagnostic Trouble Codes (DTCs). KWP2000 defines several services that access DTCs based on their group or status.

Input/Output Control

KWP2000 defines services to modify internal or external ECU signals. One example is redirecting ECU sensor inputs to stimulated signals. The control parameters of these commands are manufacturer specific and not defined in the standard.

How to Get a Bigger, Stronger and Longer Penis Without Pills, Surgery Or Devices

As a woman let me tell you that yes size absolutely does matter. This is not to be condescending, but size does make a huge difference (no pun intended).

But let me stress that I would highly advise against getting surgery, pumps and taking pills to make your penis muscle bigger. All those methods are costly and gimmicks at best.

You may be laughing or wondering, “You want me to do penis exercises? Like put a little dumbbell or weights on my penis?” Fortunately for you exercising your penis is a safe and very easy method to increase your penis length and girth. It doesn’t require any weights or devices.

Like any other muscle of your body, you need to exercise your penis muscle. If you don’t exercise your muscles you know what happens you become weak, and flabby. I’ll give you an example of a popular penis exercise for beginners called jelqing.

Wet Jelqing

Before you do this exercise, as with any exercise, you must warm up. To warm up simply take a nice hot shower or use a warm towel of place it on your penis and the surrounding area.

You will need lubrication or oil; I recommend using olive oil, coconut oil, grape-seed oil or almond oil. Your penis needs to be 20-40% erect to do this. If you find yourself getting aroused, stop and then wait till your penis is again 20-40% erect.

Put oil all over your hands and your penis. Then make a circle with your index finger and your thumb at the base of your penis. Slowly run your two fingers down the length of your penis until you reach the end of the shaft, just under the head. Repeat with your left hand. Do 15 sets totaling 30 strokes each hand.

See it’s that easy. In two months time you will find your penis bigger, stronger and longer and you will have more control over it preventing premature ejaculation.

The Best Treatment For the Inattentive ADHD and ADD

The best medicine for ADHD Inattentive (ADHD-PI) or Inattentive ADD is probably not Ritalin. There are some psychiatrists that believe that Inattentive ADD or ADHD-PI may not be ADHD at all and that patients with this subtype of ADHD may respond totally differently than the other subtypes to stimulant medication.

Dr. Russell Barkley, a foremost authority on ADHD, has written this about treating the inattentive subtype of ADHD: “These children do not respond to stimulants anywhere near as well as AD/HD hyperactive, impulsive children do. Only about one in five of these children will show a sufficiently therapeutic response to maintain them on medication after an initial period of titration. Oh, you’ll find that about two-thirds of them show mild improvement, but those improvements are not enough to justify calling them clinical responders, therapeutic responders. Ninety-two percent of AD/HD children respond to stimulants. Twenty percent of these children respond to stimulants. And the dosing is different. AD/HD children tend to be better on moderate to high doses. Inattentive children, if they’re going to respond at all, it’s at very light doses, small doses. “

Many people with ADHD-PI however, successfully use stimulant treatment. For some people even a mild improvement is reason enough to stay on the medication. The best medicine is always tailored to each individual but there is data that Ritalin works less well for ADHD-PI than the Adderall family of drugs. It would be nice, I think, to have an easy and quick reference for the medicines used to treat for Primarily Inattentive ADHD. To better understand the treatments available, a very basic lesson in the biology of ADHD is handy.

All types of ADHD are thought to be the result of a problem with neurotransmitters in our brain. The two neurotransmitters that cause the major symptoms of ADHD are Dopamine and Epinephrine (norepinephrine). Very simply stated, a low amount of dopamine in our brain will cause hyperactivity and impulsivity. A low amount of epinephrine will cause lack of focus, lethargy, and mental fatigue. Serotonin, another neurotransmitter, is secreted and depleted in tandem with dopamine and epinephrine. Low levels of serotonin make us irritable, tired, and depressed. The level of circulating serotonin is related to the levels of these other two neurotransmitters and vice versa.

All the treatments for ADHD work to optimize neurotransmitter function in the brain. The Ritalin (Methylphenidate) family works by increasing both brain dopamine and epinephrine. The Adderall (Amphetamine) family works by increasing brain epinephrine and dopamine but this family of drugs increases dopamine by only about half as much as Methylphenidate. Strattera (atomoxetine) works by increasing brain norepinephrine. Guanfacine (Intuniv) regulates the flow and effectiveness of neurotransmitter receptors in the brain in a way that reduces hyperactivity, improves working memory, and diminishes impulsivity, and distractibility.

So what is the best medicine for ADHD-PI? Some doctors believe that the Adderall family works better than the Ritalin family for the inattentives as the medication’s effects on norepinephrine are greater than the effects on dopamine. All stimulants can make some inattentives anxious but ADHD-PI patients seem to tolerate the amphetamines better than they tolerate the Ritalin family.

It would seem that using this very simple understanding of the biology of ADHD, that Strattera should work the best for ADHD-PI. The reality is that Strattera only works for some patients with ADHD-PI. We are not sure why this is the case. It would also seem that Intuniv would work poorly for folks with ADHD-PI as its main effect is on hyperactivity. The reality is that some doctors have found that Intuniv works well for ADHD-PI patients.

Because some folks with ADHD also have problems with depression and anxiety, antidepressants such as the tricyclics (Elavil, Norpramin, Tofranil) which work on Norepinephrine and Serotonin are sometimes prescribed. There are many patients who report relief of their inattentive symptoms when they are treated with the tricyclics. The SSRIs, the selective serotonin reuptake inhibitors, (Prozac, Paxil, Celexa) work only on serotonin and are sometimes used in conjunction with the stimulants to treat ADHD-PI with depression but may also help inattention.

All of these medications can have side effects. The stimulants can cause weight loss, anxiety, and sleep issues. Intuniv can cause drops in blood pressure and sleepiness, Strattera can cause sexual dysfunction and has as do the SSRIs a warning for pediatric patients regarding an increased risk of agitation, irritability and suicidal thinking. Usually the medication side effects are mild or rare but when they are not, they will unfortunately dictate what treatment can be used.

Some researchers believe that once we have mapped the genetic issues involved in ADHD, we will be able to tailor individual treatments for everyone with ADHD. This is a tall order as the neurotransmitter actions in the brain are complex and none of the …

Parasites-How Do You Know if You Have Worms and What to Do About Them

I had a client a while back who had this question for me: “I’ve noticed that I’ve been getting menstrual cramps the last few months and I don’t usually have them. Can you tell me what you think it is?”

It was determined, strangely or not so strangely enough, that the cause of those menstrual cramps was parasites! And this isn’t the only disease caused, in part, by parasites. Read on…

I recommend for her, and for you, that you worm both your pets and yourselves about twice a year.

I’ve tried natural wormers over the years–actually LOTS of natural wormers such as: garlic, cloves, black walnut, cilantro, wormwood, pumpkin seeds, pomegranate, diatomaceous earth, Rascal, Zymex, Vermifuge, several other natural wormer formulas and homeopathics, but nothing seems that effective. I’ve even tried the Hulda Clarke “Zapper” (which does not work by the way, because the voltage of the thing is not strong enough to penetrate the fecal material within the bowels–save your money by not purchasing or building one!) I am sorry to report (as a naturopath) that studies show these herbal treatments are only about 20% effective. The Certified Organic organization says when all else fails, use ivermectin (which is not organic in any way), but have you seen the list of side affects to that particular product? Three pages long!

I will say that when a client comes to me, I ask them if they have any cravings. One sure sign they are “wormy” is when they tell me they crave cloves. Clove oil has been shown to penetrate the egg and kill it where the other herbs only kill the adults. Herbs that kill parasites are often found in herbal cancer formulas because parasites are often a core cause of that particular disease.

One reason why I push so much for routine worming is that the medical field often misses this diagnosis–especially if you don’t have a history of traveling outside the United States where parasites are often a big issue. I love working in my garden and often I am an open-mouth breather when I’m working up the soil. We also live in a windy area and dirt from the wheat and grass fields is often in the air. I try to wash my hands frequently, but anyone who works in the dirt knows that it’s nearly impossible to keep from getting it on you.

Another reason why I stay on top of the worm situation is that worms carry viruses into our body. Some of these viruses can cause cancer, Parkinson’s, multiple sclerosis, and a host of other horrid diseases.

In my opinion, the safest, cheapest, easiest, most effective way to worm yourself and your pets of the most common worms (roundworms–also called strongyles, thread worms, pinworms, ascarids) is to go to the local feed store and get a bottle of horse or dog roundwormer–Equi-Phar, ProTal, Strongid-T, and Nemex are all trade names of Pyrantel Pamoate. I say this because when you ask your doctor for a wormer, they usually tell you there is no need for you to be wormed. Make sure it says Pyrantel Pamoate on the label and nothing else added. Combination wormers are not always needed and not always safe.

Dosage: Use the dose of 5-10 mg/kilogram bodyweight (about 1 ml/5 to 10 pounds of a 50mg/ml solution) of bodyweight for dogs and humans and 1 ½ ml/5 to 10 pounds for cats. One teaspoon is 5 ml or cc (ml and cc are the same unit of volume). If you are not good at math, you’ll want to double-check that you are taking the correct dosage. Although this wormer is so safe that you can take 40 times the amount with no harmful side effects, you don’t want to under dose. And, “If a little is good, a LOT is not necessary better.” The most common side effect is nausea so take this product with something on your stomach like a small meal or a few crackers.

One dose is all it takes, but you may have to give a second dose 7-10 days after the first dose for the initial worming or if the wormload is quite large. I use this once a year now for myself as I have had a history of heart palpitations (another symptom associated with parasites.) The worms will be dead within 20 minutes. You won’t feel anything and should have no side-effects. You won’t see anything pass into the toilet as a general rule but if you are wondering about what is passing, a photo often helps. Often people will mistake intestinal shreds for flatworms or tapeworms and undigested food for parasites. This wormer will not kill those types of parasties. Healthy Nutrition Beats Stress at Its Own Game

Did you know that stress can be deadly? While most of the everyday stress experienced from time to time by most of us is not dangerous in and of itself, prolonged periods of stress have been known to cause serious health problems.

Stress is caused by many factors and affects everyone a bit differently. Because there are so many variables at play, each individual will have better success with certain strategies than others. Learning to recognize stress is the first step in the process of finding a way to deal with and reduce it.

Although there are many techniques that can and should be used to effectively deal with stress, one that helps nearly every time is improved dietary nutrition. In fact, some people find that dealing with this one area of their daily routine completely eliminates their stress. In situations where the stress is caused by factors other than current dietary habits, maintaining a healthy diet is mandatory to prevent further damage.

So what exactly is proper nutrition and how does it relate to stress? Nutrition provides the body with various vitamins and minerals which have certain effects on bodily functions. Mood, which is certainly related to stress, is controlled by a chemical released in the body called serotonin. It is crucial that this chemical remain in proper balance or mood fluctuations will occur, resulting in greater stress levels.

In order to maintain a balance of serotonin it is wise to include healthy carbohydrate selections as well as foods rich in tryptophan, an amino acid used by the body to produce serotonin, in the diet. Examples of healthy carbohydrates include brown rice and other whole grains, sweet potatoes, squash and green veggies such as broccoli. Turkey is one of the best sources of tryptophan with eggs and nuts being good choices as well.

If we are honest with ourselves, we know what we should be eating. Make a conscious effort to include several fruits and vegetables with each meal and consume plenty of lean protein, whole grains and fresh water. Avoid things like fast food, sodas and sugar-laden foods. In the event you do eat one of these less desirable choices, take note of how you feel after eating it. You’ll undoubtedly find that you don’t feel quite as well as you do when your plate includes healthier selections.

That is not to say there is no place in a healthy diet for the occasional treat but such foods should be eaten in moderation and combined with more healthful foods to minimize the damage they cause. When eaten in combination with better choices they will not have the harmful effect on mood and stress levels as they would if consumed on their own.

While diet by itself will not solve every issue related to stress, it will provide a foundation of strength with which to fight back.

How And When To Use Por, Para and Porque in Spanish

Some beginner students (and advanced) usually have some problems with when they should use POR, PARA or PORQUE and it can be one of the most frustrating topics to learn in all of Spanish. In this lesson we are going to review some rules to recognize when we should use one or another.

We use POR when we want to speak about the reason behind something. In this case you have to use a noun after POR.

POR + Noun: “Me gusta Buenos Aires por el clima.” (I like Buenos Aires because of the weather.)

“Como pollo por las proteínas que tiene.” (I eat chicken because it has protein.)

In Spanish you should use PARA when you are speaking about the objective of an action or the objective of an object. In this case you will use the verb in the infinitive form. PARA + Infinitive Verb: “Ahorro dinero para viajar a otro pais”. (I save money to travel to another country.)/ “El lapiz sirve para escribir.” (The pencil is used to write.).

Finally, we use Porque when you are speaking about the reason of something, but instead of using just the infinitive of the verb, we use the conjugation of the verb.

PORQUE + Conjugated Verb: “Voy a comprarme ropa nueva porque quiero estar elegante esta noche”.

(I am going to buy new clothes because I want to be elegant tonight.)

So, when someone asks you: “Por que estudias espanol?”

(Why do you study Spanish?), you can answer in different ways:

Por mi trabajo. (Because of my job.)

Por el crecimiento del mercado latino. (Because of the growth of the Latin market.)

Para poder comunicarme con personas que hablan espanol. (To be able to communicate with people who speak Spanish.)

Para viajar por Sudamerica. (For travelling around South America.)

Porque me gusta. (Because I like it.)

Porque quiero conseguir un trabajo mejor. (Because I want to find a better job.)

This little lesson doesn’t cover all aspects of when to use POR, PARA and PORQUE, but it should give you a good start at understanding some important differences of when they are used. Be on the lookout for more lessons about POR, PARA Y PORQUE in the future.

Obesity In Childhood Ups Risk of Liver Cancer

A warning for parents of children considered obese. Obesity in childhood has been shown to lead to a higher risk of liver cancer as adults according to a recent study presented at a conference in 2012.

Liver cancer, the kind that starts in the cells of the liver, not that has spread to the liver from another site, is the third most common cancer the world over. What's worse, this form of cancer will kill almost all patients who have it within the first year. Only 10-20% of such cancers can be removed fully with surgery, chemotherapy and radiation may prove helpful, however most patients also have diseases that make these treatments harder to manage.

If that picture were not bleak enough, we also know that childhood obesity, like adult measurements, has risen dramatically over the last 30 years. The percentage of US children aged 6 to 11 years considered obese was nearly 20% in 2008, while adolescent obesity stands at 18% for the same year.

To conduct the latest work on childhood obesity and liver cancer, the team of researchers looked at birth weight and BMI at school age of over 165,000 boys and 160,000 girls in Denmark who were born between the years 1930 and 1989. Of these subjects, 252 were later diagnosed with hepatocellular carcinoma (HCC) the most common type of cancer of the liver in adults.

After examining the data, the study team calculated that by age 7 the chance of developing liver cancer as an adult went up by 12% for every 1 BMI point increase. Upon entering the teen years, the risk of hepatocellular carcinoma went up to 25% with each single BMI point. As BMI went up as a child grown, so too did the chance of being diagnosed with liver cancer as an adult. The risk was the same across genders and for all ages.

Other things known to influence liver cancer risk include alcoholism or an infection by either hepatitis B or C or other another liver disease. Yet the study results did not change when subjects who had these risk factors were taken out of the mix. This suggests that being obese in childhood was the major risk in terms of developing hepatocellular carcinoma.

Childhood obesity is known to lead to a multitude of harmful metabolic conditions, heart disease and type 2 diabetes, including fatty liver disease that may later bring liver cancer warnings experts. This is why they maintain that it is so important to keep a child's BMI in the healthy range during these years.

Losing that weight is not any easier for kids than it is for adults, but considering the risks, a healthy lifestyle should be modeled for your children. Eating a well balanced, although not entirely treat free diet is a smart start. As is being more active, not just once in a while, but on a regular basis to prevent or turn around obesity in childhood.

Teenagers – Inside the Teenage Brain

Recent research on the human brain provides parents with shocking new evidence to possibly explain the sometimes irrational, illogical and impulsive behavior of teenagers. Brain researchers can now scan the live teenage brain to observe and examine why these curious and perplexing creatures make so many impulsive and egocentric decisions, that may even sometimes lead to risky behavior.

As it turns out, brain development during the teenage years is radically more active and dynamic than previously thought. During these years, the part of the brain that requires a person to make responsible decisions, understand consequences, and process problem-solving is under heavy construction, and much of the time dysfunctional. Even though the brain is almost physically mature, the grey matter in the thinking part of the brain (pre-frontal cortex) is still making connections. So teenagers are left with most of the information reaching their brains being processed in the emotional part (limbic system).

Information processed in the limbic system, without benefit of higher level processing in the pre-frontal cortex, may result in impulsive, egocentric, and maybe even risky, behavior. Because of this ongoing construction in the thinking part of the brain, a teenager is, many times, not capable of fully processing information that is necessary to make responsible decisions. Combine this brain challenge with a teen’s temperament, maturity level, developmental stage and environmental impact, and it begins to become understandable why parents may find this time so exhausting and frustrating.

Realizing that major construction is going on inside the pre-frontal cortex of the teenage brain does not excuse inappropriate or irresponsible behavior from the teen. But understanding the teenage brain is crucial to figuring out how to interact with it. For the teenager, this time in his or her life can be a creative and emotional roller coaster ride with plenty of thrills and chills (and maybe some spills), but for parents it can be just nerve-wracking and terrifying. Healthy communication and effective discipline are what a teenager needs to help navigate this important time, especially since the brain is not yet necessarily ready or able to face all of the inevitable challenges, without support.

Each interaction with a teenager will affect development of his or her brain, helping the teen make connections in the pre-frontal cortex. During this time of heavy construction, the teenage brain needs focused and intentional support and teaching to help form and solidify these hopefully healthy connections. Parents can benefit from the understanding that there’s much work that can be done while the teenage brain in still under construction and with proper perspective and effort, a teenager can learn to be less impulsive and egocentric, and make better and more responsible decisions.

As parents decide how to more effectively communicate with the developing teenage brain, it’s vital to also consider who a child actually is, and what kind of parenting styles the child is exposed to. Most of us are the result of an even dose of nature and nurture, and understanding the nature of who a child is, and how his or her surroundings have impacted that child, can help parents formulate more effective techniques when facing challenging situations during the teenage years.

The nature of a teenager is a complex and fascinating combination of temperament, stage of development, personality, maturity level, and social connection. In addition, parents need to consider the teenager’s emotional health (self esteem) and relational health (to what degree have the teen’s closest relationships positively impacted his or her development).

And then there’s parenting styles. Healthy and effective parenting (described as authoritative), can help the positive development of the teenage brain. Using healthy communication tools like active listening, reframing, timing of teaching moments, I-messages, etc. and effective discipline tools like healthy limit setting, consequences, picking and choosing battles, few rules, etc. can greatly help the teenager’s pre-frontal cortex develop solid connections to enable responsible behavior.

There are numerous ways to test for allergies which includes EDS testing amongst other things.

The one thing you never want to jump into is taking an anti-biotic for a sinus infection until its 100% determined to be bacterial, because most often sinus infections are fungal in nature and if you do end up using a anti-biotic it can actually make the infection much worse!

If this is the case that you’ve taken anti-biotics anytime within the last several years a chronic fungal infection is often the cause of your current health problems. The next cause then of chronic sneezing is what you need to eliminate to feel better.

#3 – Candida Overgrowth: The #1 reason many people feel sick constantly for no apparent reason is something known as a Candida Overgrowth. This fungus lives inside all of us, however in some people it grows out of control.
