As Health Reform continues to unfold, the pace of change is picking up speed, as new models of reimbursement, care delivery and access are taking shape.

These shifts on the provider side should represent good news for millions of Americans that need better support from the healthcare system, especially since many of those with chronic disease are not currently getting recommended care. Coupled with the fact that most patients do not adhere to their medication and treatment regimens or recommended lifestyle and health behavior changes, and it's easy to spot some of the challenges ahead.

Here's where the payer side, particularly health plans, can really help.

Health plans have ready access to claims data and other relevant clinical information plus a direct line to their members for targeted messaging and clinical interventions. While not a traditional strategic imperative, health plans do have a significant opportunity to help drive behavior change, reduce risk, and produce positive health outcomes for their members.

Most plans already provide member ports and other helpful resources, and many have initiated to focus on enhancing these activities and initiatives to enhance their value and impact. But, to compete effectively, in this rapidly shifting market, they will need to re-orient their mindset and approach to be far more consumer-centric.

Consumer Power

After a decade of slow growth, Healthcare Consumerism has begun to take hold. Participation in high deductible health plans (HDHPs) is at a record level with 23% of all those with employer-sponsored insurance now enrolled in an HDHP. Other emerging factors such as price comparison tools, quality ratios, and retail health alternatives will continue to put more power into the hands of consumers.

As Star Ratings and other quality metrics rise in prominence, and consumer brand preference and loyalty begin to become important factors, health plans will find themselves striving for excellence in consumer experience, though outstanding member engagement.

We see a number of areas that need to be addressed in developing an effective member engagement strategy:

1. Understand how consumers think and operate in their own health and healthcare world.

Health plans need to conduct a thorough and honest review of the outreach that goes to members, along with any and all on-going interactions, the customer service function, and related areas. This is the essential starting point for meaningful change and can help uncertainty about the consumer decision process. Additionally, areas such as behavioral economics provides many important lessons and can be a good guide for creating a messaging strategy to members.

Nutrition Response Testing, a Holistic Nutrition Test, Helps Improve Health on a Cellular Level

Most people, when they first experience the work I do, find it somewhere between incredible and impossible. And objectively speaking, it is a little amazing that simply through testing someone’s muscles and reflexes, in a practice called Nutrition Response Testing (NRT), I can get clear and accurate answers about a person’s state of health. This effective, noninvasive, modality is a key tool of holistic nutrition.

NRT is a systematic type of analysis that is conducted using manual muscle testing, neurological reflexes and acupuncture points. The underlying principle is that the body is run by chemical and electrical signals that are sent from (and back to) the brain via the spinal cord and the fluid nervous system. All of our organs are controlled by the brain and nervous system, and our hundreds of muscles are controlled by and connected to the nervous system. If there is an interference with electrical messages, the result is a weakened muscle.

A healthy, unafflicted muscle is naturally strong and will resist average resistance. What you probably think of as simple muscle strength is actually a much deeper connection to nerves, organs, glands and all of the systems of the body. When I subject clients to certain substances that they may be intolerant of, then apply average resistance and the muscle response changes dramatically and go weak, I know this is a reflection of a compromise on a deeper level.

Even the gentlest of pressure will weaken the arm if there is an underlying dysfunction in an area that corresponds to a specific organ or gland. In NRT, we are interested in neurological reflexes because they are connected to the autonomic nervous system. The autonomic nervous system controls the function of our organs. It is divided into the sympathetic nervous system (think “fight or flight,” the body’s accelerator) and the parasympathetic nervous system (think “rest and digest,” the body’s brake). The neurological reflexes reflect the functioning of the delicate balance of these two systems.

That’s the underlying theory of the system. Are you with me so far? So the way NRT session works are, after talking to a patient and reviewing her symptoms, I conduct the analysis part, usually with the patient lying face-up on my table. I first establish a “baseline muscle test” of how they react when not exposed to outside agents. This is typically done using the arm and shoulder:

I will gently hold the shoulder and push the raised arm away. Because it’s not being exposed to outside influences, it will resist completely. (If there’s injury or pervasive weakness in these areas, we can employ different muscle tests.) If it tests ok, the next thing I do is have the patient put her thumb, ring and pinky fingertips together on each hand separately (with her eyes opened and closed). This reveals whether a person may have dysfunction in the parasympathetic and sympathetic nervous system.

A positive reading changes my course of the testing. Assuming it’s negative, as it is 90 percent of the time, the next step is to test for blocked regulation of the autonomic nervous system. I put my hand over three different reflex points, and I test muscle response to determine whether the patient has autonomic dysregulation. Very sick people have autonomic dysregulation to some degree. They are stuck with either the accelerator going or, the brakes on.

I also work with acupuncture points, which are related to different organs or glands. This is my direct testing technique to determine if there is a weakness in an organ or gland. I touch points throughout the body, starting with the head and working down the torso. The analogy in the conventional-medical world is hooking a patient up to a device that tests the electrical function of the heart or the conduction of specific nerves and then records the findings on a graph or a chart.

With NRT, the practitioner, for all intents and purposes, becomes the device. Instead of connecting electrodes to the specific points being tested, I contact these points with my hands. With my other hand, I test the muscle of the extended arm. If it is a problem area or is dysfunctional, the nervous system responds with weakness to the arm. The patient will feel as if her arm is becoming weak to the point, that even if she tried with all her might, she couldn’t do anything to keep her arm in a raised position.

To the practitioner, even the gentlest application of pressure causes the patient’s arm to fall. Now, let’s say I touch the acupuncture point for the kidney and liver and the patient’s muscle response is weakness (the arm drops). My next step is to place supplements on the body or in the hand and observe which nutritional supplements strengthen the weak reflex. …

If you have a choice of hospitals, inquire about their accreditation status. Hospitals are subject to inspection to make sure they are in compliance with federal standards. Policies implemented in 1989 require the release of information on request to state health departments regarding a hospital’s mortality rate, its accreditation status, and its major deficiencies.

Before checking into a hospital, you need to decide on your accommodations. Do you want to pay extra for a single room? Do you want a nonsmoker for a roommate? Do you need a special diet? Do you need a place to store refrigerated medicine? If someone will be staying with you, will they need a cot? You should try to avoid going in on a weekend when few procedures are done. When you get to your room, you should speak up immediately if it’s unacceptable.

You need to be familiar with your rights as a patient . Hospitals should provide an information booklet that includes a Patient’s Bill of Rights. The booklet will inform you that you have the right to considerate and respectful care; information about tests, drugs, and procedures; dignity; courtesy; respect; and the opportunity to make decisions, including when to leave the hospital.

You should make informed decisions. Before authorizing any procedure, patients must be informed about their medical condition, treatment options, expected risks, prognosis of the condition, and the name of the person in charge of treatment. This is called informed consent. The only times hospitals are not required to obtain informed consent are cases involving life-threatening emergencies, unconscious patients when no relatives are present, and/or compliance with the law or a court order, such as examination of sexually transmitted diseases. If you are asked to sign a consent form, you should read it first. If you want more information, you should ask before signing. If you are skeptical, you have the right to post pone the procedure and discuss it with your doctor.

Authorization of a medical procedure may be given nonverbally, such as an appearance at a doctor’s office for treatment, cooperation during the administration of tests, or failure to object when consent can be easily refused. This is called implied consent.

You need to weigh the risks of drug therapy, x-ray examinations, and laboratory tests with their expected benefits. When tests or treatments are ordered, you should ask about their purpose, possible risks, and possible actions if a test finds something wrong. For example, the injection or ingestion of x-ray dyes makes body structures more visible and greatly facilitates a physician’s ability to make a correct diagnosis. However, dyes can cause an allergic reaction that ranges from a skin rash to circulatory collapse and death. Finally, you should inquire about prescribed drugs. You should avoid taking drugs, including pain and sleeping medication, unless you feel confident of their benefits and are aware of their hazards.

The FDA and traditionally doctors do not believe that DHEA supplements taken orally do any good. That being said, the suggested way to take DHEA is 2 weeks, discontinue for 2 weeks and then repeat. Taking this supplement daily continues is detrimental.

If you have read about Yohimbe and are tempted – use caution. This herb has been associated with panic attacks, hallucinations, elevated blood pressure, headaches and dizziness. It is also bad for the kidneys.

Flavonoids (whole grains, legumes, fruits, and vegetables) are protective in coronary heart disease, stroke and cancer. Research is being done to determine if one flavonoid, chrysin, found in high concentrations in honey could inhibit the aromatase action that turns testosterone into estrogen. If it does work, that would increase the level of testosterone. If it does not work, at least you are doing good things for your heart.
